Clinical Nurse Specialist

The RN Case Manager will be responsible for coordinating patient care, collaborating with healthcare professionals, and ensuring optimal patient outcomes. This role requires strong clinical assessment and critical thinking skills, as well as excellent communication and interpersonal skills.
Responsibilities:
- Conduct comprehensive patient assessments and develop individualized care plans
- Collaborate with multidisciplinary teams to coordinate patient care and achieve seamless transitions
- Provide direct and indirect nursing care to patients, including administering medications, treatments, and procedures
- Educate patients and their families on disease management and treatment options to promote informed decision-making
- Coordinate communication between healthcare providers and community resources to enhance patient care
- Monitor patient progress and adjust care plans as necessary to ensure the best possible outcomes
Requirements:
- Hold an active and unrestricted RN license in the state of Maine
- Prior experience in case management or a related field is highly desirable
- Demonstrate strong clinical assessment and critical thinking skills to inform decision-making
