Job Summary
We're seeking a compassionate Patient Care Specialist to join our team. This role involves caring for residents under the direction of a Registered Nurse (RN), LPN/LVN, or Director of Nursing.
Responsibilities
• Take and record vital signs
• Measure and record height, weight, and fluid intake/output
• Recognize abnormal changes in body functioning and report them to a supervisor
• Assist with medication reminders, bathing, grooming, dressing, escort service, and other activities of daily living
• Transfer, position, and turn residents
• Provide skin care to residents
• Contribute to resident assessment and plan of care
• Provide nursing rehabilitation/restorative nursing services
• Follow the schedule of resident needs set by supervisor
• Provide emotional and social support to residents
• Promote and protect resident rights, assist in informed decision-making, treat with dignity and respect, protect personal belongings, report suspected abuse or neglect, avoid physical restraints, and support independent expression and choice
• Inform supervisor of resident issues or concerns
• Respect and encourage resident independence and dignity
• Respect confidentiality
Requirements
• A High School Diploma or General Education Degree
• 1+ years of clinical/nursing experience
• CNA licensure (dependent on state regulations)
• Acceptable proficiency in English for job duties, communication, and emergency response
• Basic computer skills
• Proficient verbal and written communication skills
• Dedicated, compassionate, and energetic
