Job Summary

We are seeking a highly skilled Primary Care Clinician/PA/NP to join our team in Auburn, ME. As a Primary Care Clinician/PA/NP, you will conduct adult/geriatric assessments, medical history reviews, diagnoses, treatment plans, health education, physician referrals, and case management referrals. You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.

This is a PRN/part-time position offering flexible scheduling, competitive pay ($95 per completed assessment), and mileage reimbursement. The ideal candidate will have a Master's degree in Nursing or a related field, current RN and NP/PA licensure, board certification, and a minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.

Responsibilities
  • Conduct comprehensive adult/geriatric assessments, including medical history reviews, diagnosis, treatment planning, health education, and referrals as needed.
  • Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel to ensure comprehensive care and support.
  • Provide services in a variety of settings, including home visits, skilled facility visits, and telehealth visits.
Qualifications
  • Master's degree in Nursing or a related field.
  • Current RN and NP/PA licensure in the state of practice, including prescription authority.
  • Board certification in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner/Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
  • A valid state driver's license and ability to drive a car.
