Job Title: Home Health Admissions Registered Nurse

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Home Health Admissions Registered Nurse to join our team. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care in the comfort of patients' homes.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments, identify patient needs, and provide education on disease management and self-care.
  • Coordinate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Provide skilled nursing care, including wound care, infusion therapy, and catheter management.
  • Collaborate with primary caregivers to ensure optimal patient functioning and independence.

  • Active RN license in the state of application
  • Valid CPR certification

Preferred Qualifications:

  • Medicare skilled nursing experience
  • Basic understanding of Oasis
  • 1-year RN experience in a healthcare setting
