Home Health Admissions Registered Nurse

Aveanna Healthcare is seeking a skilled and compassionate Home Health Admissions Registered Nurse to join our team. As a key member of our clinical team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.
Key Responsibilities- Develop and implement individualized care plans in accordance with physician orders and patient needs.
- Educate patients and their families on disease management, medication adherence, and self-care techniques.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Provide skilled nursing care, including wound care, infusion therapy, and catheter management, in patients' homes.
- Active RN license in the state of application.
- Valid CPR certification.
- 1 year of experience in a healthcare setting, preferably in home health or skilled nursing.
- Basic understanding of OASIS and Medicare guidelines.
