Job Description

We are seeking a skilled Nurse Care Specialist to join our team at WMCHealth.

About the Role:

  • To provide exceptional patient care and support systems in partnership with the Clinical Manager and interdisciplinary team.
  • To demonstrate autonomy in practice by performing thorough and ongoing holistic nursing assessments, identifying and prioritizing current and potential problems, and developing individualized plans of care.
  • To collaborate with patients, their support systems, and healthcare team members to meet present and discharge needs, implementing and revising plans of care as necessary.
  • To exercise independent judgment and critical thinking skills to provide individualized teaching to patients and their support systems throughout the continuum of care.
  • To communicate effectively with the interdisciplinary healthcare team, utilizing active listening, relationship building, and assertive communication techniques.

Required Skills and Qualifications:

  • A graduate degree in nursing from an accredited school.
  • Current licensure in the State of New York or permittee.
  • Basic knowledge of nursing theory and practice.
  • Knowledge of Hospital structure, objectives, and standards of practice.
  • Positive communication skills.

Estimated Salary Range: $60,000 - $80,000 per year.
