Job Title: RN Case Manager

At Heartlinks, we are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will play a vital role in providing high-quality hospice care to patients and their families.

Job Summary:

The RN Case Manager will be responsible for planning, organizing, and directing hospice care services. This includes conducting comprehensive assessments, developing individualized care plans, and coordinating patient care with the interdisciplinary team.

Key Responsibilities:
  • Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families.
  • Coordinate patient care with the interdisciplinary team, including physicians, nurses, social workers, and other healthcare professionals.
  • Provide education and support to patients and families to promote self-care and independence.
  • Act as a liaison between patients, families, and healthcare providers to ensure seamless care coordination.
Requirements:
  • Current licensure as a Registered Nurse in the state of Washington.
  • Graduate of an accredited school of nursing.
  • Minimum of two years' experience in hospice or home care nursing.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
