Hospice Case Manager Job Summary

At Heartlinks, we are seeking a skilled RN Case Manager to join our team. As a Hospice Case Manager, you will play a vital role in planning, organizing, and directing hospice care for patients in their homes and communities.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Provide education and support to patients and families to promote self-sufficiency and comfort care.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination.
  • Act as a liaison between patients, families, and healthcare providers to ensure effective communication and care.
Requirements:
  • Current licensure to practice professional nursing in the state of Washington.
  • Graduate of an accredited school of nursing.
  • Minimum of two years' experience in public health, home care, or hospice nursing.
  • Excellent observation, problem-solving, verbal and written communication skills.
  • Ability to work independently with minimal supervision.
