Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our team at Advocate Health. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working collaboratively with other healthcare professionals to achieve optimal patient outcomes.
Key Responsibilities- Assess, diagnose, and develop treatment plans for patients, taking into account their age, acuity, and clinical condition.
- Order, perform, and interpret diagnostic studies, and prescribe pharmacologic and non-pharmacologic interventions and therapies as needed.
- Manage patient conditions based on clinical indication, evidence-based care, cost-effectiveness, and risk-benefit analysis.
- Provide health promotion, disease prevention, and disease management counseling and education to patients and families.
- Collaborate with other healthcare team members to ensure consistent, coordinated care and clear communication.
- Document patient information accurately, completely, and in a timely manner in the electronic medical record.
- Participate in quality, safety, and peer review initiatives, as well as organizational and departmental meetings and committees.
- Active RN and APRN/APRN-FPA/APNP license(s) in state(s) of practice.
- Active national board certification in area of clinical practice and populations served.
- Active DEA registration prior to hire.
- Active BLS and/or ACLS, PALS, NRP certification as required by clinical practice.
- Demonstrated high level of clinical proficiency and excellent decision-making skills.
- Ability to work independently and as an effective member of a healthcare team.
- Adaptability to evolving technology and proficiency with the electronic medical record.
- Excellent communication and collaboration skills.
This role requires the ability to stand, walk, bend, stoop, and twist continuously throughout the workday. The incumbent must also have functional speech, vision, touch, and hearing. Additionally, the ability to lift up to 50 lbs from the floor to waist, lift up to 20 lbs over the head, and carry up to 40 lbs a reasonable distance is required.
