The Community Outreach Coordinator plays a pivotal role in managing all external partnerships, including but not limited to hospitals, healthcare facilities, and medical providers. This position is essential in fostering communication and resolving issues between these external entities and our organization. The coordinator is tasked with meeting specific targets related to the volume and type of admissions, aligning with the agency's financial goals and strategic objectives.
This role requires travel within the designated service area. Ideal candidates will reside within a reasonable commuting distance to our operational hub.

Key Responsibilities:
- Stay informed about all programs and services offered by Lake Sunapee VNA & Hospice, and represent the organization effectively
- Support efforts to enhance communication with healthcare providers and the community
- Facilitate educational initiatives for external partners regarding our services
- Meet admission targets as set by supervisory staff
- Keep internal teams updated on community changes and competitor activities that may affect service demand
- Monitor and assess community expectations and satisfaction levels, providing feedback to relevant personnel
- Assist in the referral process and ensure smooth transitions for patients accessing our services
- Collect and share necessary data for maintaining accurate client records
- Identify needs for promotional materials and assist in their development
- Participate in required training and professional development opportunities
- Proactively identify challenges and propose constructive solutions
- Prepare and submit reports as requested
- Engage in respectful communication with team members, clients, and the community
- Adhere to organizational policies and procedures
- Perform additional duties as assigned
Qualifications:
- Preferred experience in marketing or sales
- Background in healthcare settings is advantageous
- Homecare experience is a plus
- Current RN license in New Hampshire is required
- Strong written and verbal communication skills are essential
- Proof of flu vaccination is necessary
- Reliable transportation and an active driver's license are required
- Automobile insurance and a recent physical examination are mandatory
- TB shot documentation is required
