**Job Title:** Health Center Coordinator, Dearborn Park

**Job Summary:** We are seeking a highly organized and detail-oriented Health Center Coordinator to join our team at Neighborcare Health. The successful candidate will provide administrative, outreach, and service coordination support to the School-based Health Center.

Key Responsibilities:
  • Administrative: Coordinate schedules of all providers in the Health Center, ensuring all students arrive for appointments, are checked in appropriately, and are seen according to their needs and the needs of the providers.
  • Outreach: Coordinate and lead targeted outreach activities focused on promoting, integrating, and informing students, families, school staff, and the general school community about the School-based Health Center services.
  • Service Coordination: Serve as the primary point of contact for referrals and service inquiries from school staff and families for School-based Health Center services.
Requirements:
  • High School diploma or equivalent
  • Administrative work experience or experience at a high volume medical office
  • Experience in social service outreach
