Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments of patients in their homes, identifying potential health risks and providing personalized care recommendations.
Key Responsibilities- Conduct thorough health assessments, including medical history, physical examination, and laboratory tests, to identify potential health risks and develop personalized care plans.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and optimal health outcomes.
- Develop and implement effective care plans, including medication management, disease management, and health education, to address identified health risks.
- Provide high-quality patient care, demonstrating empathy, compassion, and cultural sensitivity in all interactions.
- Stay up-to-date with the latest clinical guidelines, research, and best practices in healthcare to ensure the highest level of care for patients.
- Master's degree in Nursing or Physician Assistant Studies, or equivalent experience and licensure.
- Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACN in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant, with a strong track record of providing high-quality patient care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound clinical decisions.
- Competitive hourly rate of $95.00 per completed assessment.
