About the Position

We are seeking a Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Clinician. In this role, you will conduct comprehensive health assessments in the patient's home, reviewing their medical history, medication adherence, social environment, and other risks.

As a Home Risk Assessment Clinician, you will work collaboratively with physicians, case managers, and other healthcare professionals to develop personalized care plans that improve patient outcomes.

Responsibilities
  • Conduct comprehensive health assessments in the patient's home
  • Review medical history, medication adherence, social environment, and other risks
  • Develop personalized care plans with physicians, case managers, and other healthcare professionals
  • Collaborate with patients, families, and caregivers to improve patient outcomes
Requirements
  • Master's degree in nursing or a related field
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
Travel Requirements

As a Home Risk Assessment Clinician, you will be required to travel to patients' homes and other locations to conduct assessments. You must have a valid state driver's license and be able to drive a car. Proof of adequate automobile insurance coverage is also required.
