Key Responsibilities:- Coordinate patient care from pre-transplant to post-transplant, ensuring timely and effective communication with the transplant team, patients, and families.
- Develop and implement individualized care plans, taking into account the unique needs and circumstances of each patient.
- Collaborate with the transplant team to ensure accurate and timely documentation of patient information, including medical history, test results, and treatment plans.
- Provide education and support to patients and families, addressing their concerns and questions throughout the transplant process.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving outcomes.
- Stay up-to-date with the latest developments in lung transplant care, incorporating new knowledge and best practices into your work.
- Graduate of an approved school of nursing with current registration in Massachusetts.
- At least 2 years of experience in pulmonary, thoracic surgery, or transplant nursing, with a strong background in ambulatory care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
