About the Role

We are seeking a highly skilled Lung Transplant Nurse Coordinator to join our team at Mass General Brigham. As a key member of our transplant program, you will be responsible for coordinating the post-transplant care of patients, ensuring seamless transitions between inpatient and outpatient settings.

Key responsibilities include:

  • Coordinating patient care plans, including medication management and follow-up appointments
  • Communicating with patients, families, and healthcare teams to ensure effective care coordination
  • Collaborating with multidisciplinary teams to develop and implement patient care plans
  • Providing education and support to patients and families on post-transplant care and management

Requirements

  • Graduate of an approved school of nursing with current registration in Massachusetts
  • 2+ years of experience in pulmonary, thoracic surgery, or transplant nursing
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
