About the Job

We are seeking an experienced RN Clinical Manager to join our Hospice team in Chesapeake, VA. As a key member of our clinical leadership team, you will be responsible for providing high-quality patient care and overseeing the delivery of compassionate end-of-life services.

Job Summary

This is a unique opportunity to make a meaningful difference in the lives of individuals facing life-limiting illnesses. You will work closely with our interdisciplinary team to develop and implement personalized care plans, provide education and support to patients and families, and collaborate with healthcare professionals to ensure seamless transitions of care.

Key Responsibilities
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families
  • Provide direct patient care, including assessments, evaluations, and interventions
  • Collaborate with the hospice team to ensure comprehensive care and effective communication
  • Recruit, educate, and supervise hospice staff to promote a culture of excellence and compassion
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements

To be considered for this role, you must have:

  • A current RN license in the state of Virginia
  • At least one year of experience in hospice or palliative care
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a collaborative team
  • Strong problem-solving and critical thinking skills
