Salary and Benefits

Payouts for this position will be determined by years of experience and location. Job Description

You will provide psychosocial evaluations and ongoing counsel to patients and families during their end-of-life journey, working according to the plan of care. Emotional support to patients and families is also required when it's needed most.

Qualifications

To be successful in this role, you should have a heart to serve patients and families, with a passion for providing the best possible care. A minimum MS degree in social work from an accredited school of social work approved by the Council of Social Work Education is required, along with current state licensure as a social worker. Clinical work experience, preferably in healthcare or hospice, is also preferred. Reliable transportation, ability to sit, stand, bend, move intermittently, and lift at least 25lbs are necessary physical requirements.
