ICMS Case Manager

Wellnest Emotional Health & Wellness is seeking a highly skilled ICMS Case Manager to join our team. As a key member of our multi-disciplinary field-based team, you will work closely with clinicians, case managers, employment specialists, and community partners to provide comprehensive support to residents living at The Nest on Florence.
Job Summary:The ICMS Case Manager will work with a healing-centered, trauma-informed approach to develop, implement, and monitor service/treatment plans for residents. You will provide individualized support to help residents achieve their personal, social, emotional, mental health, educational, medical, housing, physical, and financial goals.
Key Responsibilities:- Formulate individualized service plans that identify residents' needs and goals
- Mediate and advocate on behalf of clients to obtain and maintain suitable housing, healthcare, jobs, and mental health services
- Search the community for additional needed or desired services and present options to residents
- Connect residents to resources such as health insurance, housing, low-cost childcare, chemical dependency services, education, mental health services, medical care, income (SSI, GR), and employment
- Provide field-based support to residents, including transportation to resources and appointments
- Attend and participate in Care Coordination Meetings and document services in electronic online databases
- Collaborate with participants and other service providers to maintain housing stability goals
- Bachelor's degree in social work or related field with an emphasis in community mental health work
- One year of experience working with homeless individuals or two years of experience providing direct mental health or intensive case management services
- Experience working with clients with mental illness, chronic health issues, and substance use disorders
- Knowledge of surrounding poverty, homelessness, substance use, and social factors related to each dynamic
- Ability to work a flexible schedule, including some evenings and weekends
- Annualized Salary Range: $47,098 to $52,919
- Eligibility for an annual $3,000 bilingual Spanish differential
