About the Role

We are seeking a skilled Registered Nurse (RN) to join our team as a Care Manager for elderly patients. In this role, you will provide comprehensive care management, skilled nursing, and coordination of services for patients with complex medical needs.

Responsibilities:
  • Coordinate healthcare services to enhance patient independence and quality of life
  • Travel to various locations to provide personalized and proactive care
  • Work collaboratively with an interdisciplinary team to develop and implement patient-centered care plans
  • Assess patient needs and coordinate access to appropriate care and services

Requirements

To be successful in this role, you will need:

  • A current Oregon Registered Nurse License
  • National Provider BLS certification within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
