Job Summary:

As a Care Manager RN, you will provide patient-centered care coordination services to patients and families, assess patients' needs, and develop individualized care plans.

Responsibilities:

  • Provide patient-centered care coordination services.
  • Assess patients' needs and develop individualized care plans.
  • Collaborate with healthcare teams to ensure seamless transitions of care.
  • Communicate effectively with patients, families, and healthcare providers.

Requirements:

  • Nursing degree/diploma required upon hire.
  • Bachelor's degree in Nursing or higher within 3 years of hire.
  • Oregon Registered Nurse License upon hire.
  • 2 years of Acute care hospital or community-based experience.
  • 1 year of experience with electronic medical records or documentation systems.
  • 1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.

Salary Range:

$49.77 - $73.13 per hour, with additional compensation available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
