Hospice Physician/Nurse Practitioner Representative Opportunities

**Job Summary**
The Hospice Physician/Nurse Practitioner Representative will play a crucial role in providing medical direction and support to our Interdisciplinary Group (IDG) team. This includes supervising Face-to-Face encounters, participating in IDG meetings, monitoring compliance with policies and procedures, and communicating with the Medical Director.
**Key Responsibilities:**
- Supervise the planning, coordination, and delivery of Face-to-Face encounters to ensure accurate assessments and effective care planning.
- Participate in IDG meetings and contribute expertise on Face-to-Face findings to inform patient care decisions.
- Monitor and maintain compliance with company policies and procedures to ensure high-quality patient care.
- Communicate effectively with the Medical Director regarding Face-to-Face encounters and further evaluation needs.
**Requirements:**
- A Bachelor's degree in Nursing or a related field is required for this role.
- A current valid driver's license and reliable transportation are necessary for travel to patient sites.
- The ability to work independently, make judgments based on assessments and data available, and prioritize tasks effectively is essential.
