Job Title: Care Coordinator
We are seeking a highly organized and detail-oriented Care Coordinator to join our team. The successful candidate will be responsible for coordinating patient care, ensuring compliance with clinical protocols, and working effectively with patients, family members, physicians, and staff.
Key Responsibilities:
- Perform intake on all referrals upon receiving them, ensuring accurate and timely processing.
- Schedule referrals for consultation and/or assessment on patients with pressure ulcers and chronic wounds, prioritizing patient needs and care.
- Review charts of patients with wounds to ensure accurate documentation and treatment, maintaining high standards of care.
- Participate in clinical outcome monitoring, follow-up, and agency performance improvement initiatives, driving continuous quality improvement.
- Collaborate with the Medical Director to develop plans for discharge and implement standards of care, ensuring seamless patient transitions.
- Educate patients and family members on Advanced Wound Therapy and its interplay with current conventional wound care, promoting informed decision-making.
- Act as a consultant to the Medical Director and wound care providers, educating staff on products available for use in the program, and promoting best practices.
Requirements:
- Excellent organizational and prioritizing skills, with a focus on delivering high-quality patient care.
- Effective and professional verbal and written communication skills, with the ability to communicate complex information to patients and staff.
- Nursing Degree from an accredited college or university, or is a graduate of an approved RN or LPN program, with active licensure to practice in Pennsylvania.
- Strong attention to detail and excellent organization and time-management skills, with the ability to work in a fast-paced environment.
- Ability to speak effectively before groups of customers or associates of the organization, with excellent presentation and communication skills.
