Job Summary

We are seeking a highly skilled Clinical Liaison to join our team at PAM Health. As a Clinical Liaison, you will play a critical role in cultivating referral relationships, managing assigned territories, and completing patient assessments. You will work collaboratively with our business development team members, nursing staff, and other internal and external stakeholders to facilitate referral conversions and provide exceptional patient care.

Key Responsibilities
  • Develop and maintain strong relationships with referral sources, physicians, and other healthcare professionals to drive business growth and improve patient outcomes.
  • Conduct on-site pre-admission assessments, phone/FAX assessments, and document pre-admission assessments accurately and completely in the approved format.
  • Review medical records, identify pre-certification financial coverage and admission criteria, and serve as a liaison to the Admissions office for insurance companies.
  • Communicate with facility Admissions department and facility CEO daily to optimize census management and case finding strategies.
  • Follow up on all pending and medical denials in person to re-evaluate potential for admission.
  • Conduct follow-up with referral facilities to ensure satisfaction and service levels were met.
  • Design and manage marketing strategies for assigned territories, utilize sales and marketing techniques to show growth in admission and revenue volume, and create Quarterly Business Development Plans.
  • Identify contacts and manage potential referral sources using account management systems, utilize prospecting methods to generate leads, and meet referral and admission goals for assigned territories.
  • Participate in community events and activities, attend weekly Business Development meetings, and provide educational presentations to referral sources, families, and community members as requested.
Requirements
  • Degree from an accredited college or university in a health field, preferably nursing, with current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy.
  • Minimum of two years' experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.
  • Excellent clinical and assessment skills, effective presentation and training skills, excellent written and oral communication skills, and effective crisis management skills.
  • Knowledge of PPS payment/reimbursement systems and DRG's/case weights, patient rights, and commitment to patient advocacy.
  • Ability to collect, document, and analyze data, word processing and data entry skills, and network access to community, state, and national resources and health services organizations.
