PAM Health is seeking a highly skilled Clinical Navigator to join our team. As a Clinical Navigator, you will play a critical role in cultivating referral relationships, managing assigned territories, and completing patient assessments.

Key Responsibilities

Pre-Admission Assessment

  • Develop and maintain responsibility for patient referrals from initial contact to admission.
  • Conduct on-site pre-admission assessments to evaluate patient suitability for our services.
  • Review medical records to identify pre-certification, financial coverage, and admission criteria.
  • Communicate admission status to referral sources and ensure timely follow-up on pending cases.

Business Development

  • Design and implement marketing strategies to drive growth in admission and revenue volume.
  • Utilize sales and marketing techniques to promote our services and build strong relationships with referral sources.
  • Develop and maintain a Quarterly Business Development Plan to ensure alignment with company goals.

Education and Outreach

  • Identify and educate new referral sources about our programs and services.
  • Schedule educational inservices to promote our services and build relationships with referral sources.
  • Coordinate facility tours and gather information on healthcare providers in the service area.

Documentation and Reporting

  • Maintain a log of all pending cases and provide regular updates on activities and market trends.
  • Maintain a resource library of articles and information for distribution to referral sources.
  • Complete a quarterly plan and maintain territory Account Management System.
Requirements

Education and Training: Degree in a health field, current licensure in nursing or respiratory therapy preferred, prior marketing and/or rehabilitation experience preferred.

Experience: Minimum of two years' experience in the designated field of license, preferably with rehabilitation and marketing experience.

Knowledge, Skills, and Abilities:

  • Excellent clinical and assessment skills.
  • Effective presentation and training skills.
  • Excellent written and oral communication skills.
  • Effective team leadership skills and knowledge of payment/reimbursement systems.
  • Ability to collect, document, and analyze data.
  • Strong relationship management skills with referral sources and physicians.
