Job Summary

We are seeking a skilled and compassionate RN - Hospice to join our team at PeaceHealth. As a Hospice Nurse, you will provide direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions.

Responsibilities
  • Perform initial evaluation of patient/home and apply admission criteria to determine if patient is a safe candidate for home health/hospice services.
  • Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
  • Develop, coordinate, modify, and manage written individualized plan of care, as ordered by the primary physician, to meet patient's long and short-term medical goals and appropriate/timely home health outcomes.
  • Develop and clinically manage the Home Health Aide's care plan according to the patient's evolving personal care needs.
  • Coordinate care, collaborate, and communicate with other members of the interdisciplinary team to ensure appropriate and smooth flow of care/services through the care continuum.
Requirements
  • Graduate of an accredited school of nursing required.
  • BSN preferred.
  • Two years acute clinical or home health/hospice nursing experience required.
  • Current Registered Nurse licensure in the state in which the nurse will be practicing.
  • Current BLS (Basic Life Support) certification required at date of hire.
  • Valid driver's license with an automobile that is insured to state regulations and is in good running order.

PeaceHealth requires a completed Primary Vaccine Series (e.g., 2 dose monovalent Pfizer, Moderna or Novavax series or 1 dose J&J vaccine series) or be Up to Date (receiving the most recent Pfizer/Moderna bivalent vaccine/booster) for COVID-19 vaccination prior to their start date.
