Job Summary:

Under the direction of a Wound, Ostomy, and Continence Nurse (WOCN), the Wound Ostomy Continence Nurse Trainee will provide direct care to patients with gastrointestinal and genitourinary stomas, gastrointestinal and genitourinary continence disorders, acute/chronic wounds, and fistulas. This role will encompass the complete care continuum, including assessment, testing, planning of care, interventions, and ongoing re-evaluation of patients.

Key Responsibilities:
  • Provide follow-up and interventions to patients diagnosed with ostomies, draining wounds, fistulae, or skin breakdown secondary to pressure, incontinence, venous hypertension, arterial insufficiency, trauma, surgery, or diabetic neuropathy and other related skin issues.
  • Provide direct patient care counseling of individuals anticipating surgery that may result in the creation of a stoma, individualized instruction for those who have an abdominal stoma for the diversion of stool or urine, or assessment and treatment recommendations for the above-stated skin breakdown conditions.
  • Assist patients through the continuum of care for management of related issues.
  • Evaluate new and currently used products for ostomy, continence, and wound care.
  • Make recommendations to patients in obtaining related products.
  • Assess individual patient needs and goals, and provide educational programs to physicians, nursing, and other paramedical professionals on ostomy/wound care management and self-care education.
  • Assist the WOCN in conducting continuing educational programs for physicians and nursing personnel, providing current standards of practice for ostomy patients.
  • Assist the WOCN in providing patient/family teaching and education regarding ostomy/wound care and self-management.
  • Consistently support compliance and the Principles of Responsibility by maintaining the privacy and confidentiality of information, protecting the assets of Kaiser Permanente, acting with ethics and integrity, reporting non-compliance, and adhering to applicable federal, state, and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente policies and procedures.

  • Accountable for consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers, and vendors.
Requirements:
  • Minimum one (1) year previous clinical nursing experience as a Registered Nurse.
  • Bachelor's degree required.
  • Acceptance into an approved, accredited Wound, Ostomy, Continence Nursing (WOCN) program.
  • Wound Ostomy Continence Nurse Certificate within 12 months of hire from the Wound, Ostomy and Continence Nursing Certification Board or Wound Ostomy Care Nurse Certificate within 12 months of hire from the Wound, Ostomy and Continence Nursing Certification Board.
  • Registered Nurse License (California).
  • Basic Life Support.
