VNS Health is seeking a highly skilled and experienced Registered Nurse to join our team as a Wound Care Specialist. As a key member of our healthcare team, you will be responsible for providing expert care to patients with complex wounds, promoting optimal healing outcomes, and collaborating with multidisciplinary teams to develop and implement effective care plans.
Key Responsibilities- Develop and implement evidence-based care plans for patients with complex wounds, taking into account individual patient needs and preferences.
- Provide expert consultations, staff development plans, and continuous quality improvement strategies to promote optimal wound care practices.
- Collaborate with multidisciplinary teams, including physicians, therapists, and other healthcare professionals, to develop and implement comprehensive care plans.
- Evaluate clinical outcomes data and analyze practice patterns to identify areas for improvement and promote organizational responses that meet the needs of patients with specific complex and chronic illnesses.
- Develop policies, standards, and protocols for the application of evidence-based practice and evaluate their impact on care management outcomes.
- Identify and develop new programs designed to improve care outcomes for complex patients, and collaborate with program leadership in implementation and evaluation.
- Educate, mentor, coach, and develop clinical staff to integrate complex illness management expertise into practice and improve quality of care.
- Perform telephonic and in-person clinical consultations as needed, and provide clinical care in accordance with VNS Health policies, practices, procedures, and Standards of Professional Practice.
- License and current registration to practice as a Registered Professional Nurse in the State of New York within 6 months of hire required.
- Valid driver's license or NYS Non-Driver photo ID card may be required as determined by operational needs.
- Clinical certification in a specific specialty, such as diabetes, oncology, wound/ostomy/continence, etc. preferred.
- Bachelor's Degree in Nursing required; Master's Degree in Nursing preferred.
- Minimum of five years clinical experience required; experience developing and delivering disease-specific educational programs preferred.
