Job Title: IDD Service Coordinator

PermiaCare is seeking a highly motivated and compassionate IDD Service Coordinator to join our team. As a key member of our staff, you will play a vital role in ensuring the well-being and quality of life for individuals with intellectual and developmental disabilities.

Job Summary:

The IDD Service Coordinator will be responsible for monitoring the care and services provided to IDD individuals, ensuring they receive the necessary support to live their best possible life. This includes assessing needs, developing service plans, facilitating service linkages, and providing crisis intervention and prevention supports.

Key Responsibilities:
  • Participate in a discovery process with individuals and their support networks to identify personal outcomes and goals.
  • Monitor progress and delivery of services to ensure individual needs and preferences are being met.
  • Document progress and satisfaction with services and receipt of services within required timeframes.
  • Meet with Legally Authorized Representatives (LARs) to monitor outcomes and services.
  • Provide assessment, service planning, monitoring, advocacy, and crisis prevention and management for assigned caseloads.
  • Remain in compliance with applicable local, state, and federal rules and regulations.
  • Advocate for individuals by reviewing rights annually and as needed, informing individuals and LARs of the complaint process, and promoting individual choice.
Requirements:
  • Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.
  • At least 1 year of experience working with individuals with intellectual and developmental disabilities or mental illness.
  • Valid Texas Driver's license, auto liability insurance, and a driving record acceptable to PermiaCare's insurance requirements.
  • Ability to maintain confidentiality of sensitive records and treatment information, client files, and protected health information in compliance with HIPAA, laws, rules, and regulations.
Preferred Qualifications:
  • QIDP certification within 6 months of hire.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability to different work environments.
Working Conditions:

This position requires the ability to work in a fast-paced environment, prioritize tasks, and maintain a high level of productivity. The IDD Service Coordinator will be required to work independently, under limited supervision, and report major activities through periodic meetings.
