We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team in Hartford and Litchfield County, CT. As a member of our interdisciplinary team, you will play a vital role in delivering high-quality home-based medical care to complex, chronic patients.
In this rewarding role, you will have the opportunity to make a meaningful difference in the lives of older adults by providing comprehensive care and promoting healthy aging. You will work closely with our team to develop and implement patient management care plans, conduct initial assessments, and provide ongoing care.

Key Responsibilities:
- Conduct initial comprehensive assessments on all newly enrolled patients and provide ongoing care thereafter
- Develop and update patient management care plans as needed, working collaboratively with the PCP and interdisciplinary team
- Prescribe appropriate diagnostics and interventions to avoid unnecessary acute admissions
- Educate patients and their legal representatives on disease processes and ways to manage them effectively
- Actively participate in ongoing meetings pertaining to patient care and clinical excellence
- Implement HEDIS measure campaigns and other quality initiatives to ensure the highest standards of care
Required Qualifications:
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Current state RN/NP or PA license
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
Preferred Qualifications:
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
- Knowledge of CPT, ICD-10, and HCPC codes
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
Salary Range: $88,000 - $173,200 annually, depending on experience. We offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution.
