We're seeking an experienced Home Health Medical Care Specialist to join our team in Hartford and Litchfield County, CT. As a member of our interdisciplinary team, you'll provide home-based medical care to complex, chronic patients, helping them access and navigate care anytime and anywhere.

As a Home Health Medical Care Specialist, you'll collaborate with primary care physicians, interdisciplinary teammates, and consultants to develop and execute patients' goals of care throughout their journey.

Key Responsibilities:

  • Conduct initial comprehensive assessments for new patients and provide ongoing care
  • Develop and update patient management care plans as needed
  • Meet with patients and their representatives to review care plans, involving primary care physicians when necessary
  • Perform basic procedures, including IV placement, blood draws, injections, and wound care
  • Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
  • Complete follow-up and post-discharge assessments according to standard operating procedure
  • Consult with hospital and emergency teams upon patient transfers
  • Educate patients on disease management and promote wellness
  • Participate in ongoing meetings to ensure high-quality patient care and clinical excellence
  • Implement HEDIS measure campaigns and quality initiatives to improve care delivery
  • Be on-call for your caseload during business hours and participate in rotational after-hours calls
  • Stay current on medical research, technology, and issues through continuing education and professional development
  • Practice in accordance with state laws and regulations governing advanced practice nursing or physician assistants

Required Qualifications:

  • Completion of a Nurse Practitioner or Physician Assistant accredited program
  • Current RN/NP or PA license in the state of Connecticut
  • Certification from the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
  • Access to reliable transportation; compliance with Optum's Motor Vehicle Safety policy

Preferred Qualifications:

  • 2+ years of clinical experience, preferably in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Familiarity with CPT, ICD-10, and HCPC codes
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
