This role involves working as a provider within the HBMC interdisciplinary team, delivering direct home-based medical care to patients within a designated geography. You'll focus on improving patients' health and well-being by collaborating with PCPs, HBMC teammates, consultants, and other healthcare professionals to develop and implement patient goals of care throughout the care journey.

Key Responsibilities:

  • Conduct comprehensive initial assessments and ongoing care for newly enrolled patients
  • Develop personalized patient management care plans and update them as necessary
  • Meet with patients or their representatives to review new or modified care plans, involving PCPs when applicable
  • Be comfortable with basic procedures, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc.
  • Prescribe diagnostics and interventions to avoid unnecessary acute admissions
  • Complete follow-up and post-discharge assessments according to standard operating procedures
  • Consult with hospital teams following patient transfers
  • Educate patients and representatives about disease processes and effective management strategies, promoting wellness
  • Participate in ongoing meetings focused on patient care and clinical excellence
  • Implement quality initiatives and HEDIS measure campaigns to ensure high standards of care
  • Be on-call during business hours and participate in rotational after-hours call programs
  • Stay current on medical research, technology, and issues through continuing education and professional development
  • Practice in accordance with state laws and regulations governing advanced practice nursing or physician assistants

Requirements:

  • Nurse Practitioner or Physician Assistant accreditation
  • Current state RN/NP or PA license
  • Certification from AANP, ANCC, or NCCPA
  • Access to reliable transportation, complying with Optum's Motor Vehicle Safety policy

Preferred Qualifications:

  • 2+ years of experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Familiarity with CPT, ICD-10, and HCPC codes
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
