Job Summary

We are seeking a skilled and compassionate Weekend Home Health RN to provide high-quality care to our patients in the comfort of their own homes. As a Weekend Home Health RN, you will be responsible for delivering comprehensive nursing services to patients with diverse needs, including those with complex medical conditions and those requiring palliative care.

Key Responsibilities
  • Conduct thorough physical and psychosocial assessments of patients to identify their unique needs and develop personalized care plans.
  • Implement and evaluate care plans, making adjustments as needed to ensure optimal patient outcomes.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
  • Document patient progress and outcomes accurately and in a timely manner, using standardized tools and templates.
  • Participate in quality improvement initiatives and contribute to the development of best practices in home health care.
Requirements
  • Current RN licensure in the state of Connecticut.
  • Minimum one year of experience in community health/homecare as a nurse.
  • Knowledge of professional nursing theory and practice, as well as skills in patient assessment, care planning, and documentation.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Valid driver's license and reliable transportation, with proof of automobile insurance.
