Weekend Nurse - Home Visits for Hospice and Palliative Patients

We are seeking a skilled and compassionate Weekend Nurse to provide home visits for hospice and palliative patients in the Eastern Connecticut area.

Key Responsibilities:
  • Conduct thorough physical and psychosocial assessments of patients to identify their unique needs and develop personalized care plans.
  • Establish and implement care plans in collaboration with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide instruction and education to patients and families on procedures, wellness practices, and disease management.
  • Monitor patient progress, adjust care plans as needed, and refer patients to appropriate therapies and resources.
  • Collaborate with healthcare teams to ensure high-quality patient care and achieve optimal patient outcomes.
Requirements:
  • Graduate of an NLN-approved school of nursing with an associate or baccalaureate degree program.
  • Current license to practice as a registered nurse in the state of Connecticut.
  • At least one year of recent experience in community health/homecare as a nurse.
Competencies:
  • Strong knowledge of professional nursing theory and practice, with the ability to apply and evaluate patient care.
  • Excellent communication and interpersonal skills, with the ability to establish and maintain effective working relationships with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Strong problem-solving and critical thinking skills, with the ability to interpret and apply guidelines and procedures.
