Key Responsibilities- Conduct comprehensive assessments of patients' physical and psychosocial needs, developing and implementing individualized care plans to achieve optimal outcomes.
- Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
- Collaborate with patients, families, and healthcare providers to ensure continuity of care and address any concerns or questions.
- Document patient progress and outcomes accurately and thoroughly, using standardized tools and templates.
- Participate in ongoing education and training to stay current on best practices, new technologies, and regulatory requirements.
- Work effectively in a fast-paced, dynamic environment, prioritizing tasks and managing time to meet deadlines and achieve goals.
- Current licensure as a Registered Nurse in the state of Connecticut.
- Minimum one year of experience in home health or community health nursing.
