Job Summary:

We are seeking a skilled RN Care Manager to join our team at the North Carolina Community Health Center Association.

About the Role:

The RN Care Manager will be responsible for coordinating and managing healthcare services for patients, ensuring seamless transitions between care settings. This role requires excellent communication and organizational skills, as well as the ability to work effectively with patients, families, and healthcare providers.

Key Responsibilities:

  • Coordinate patient care across different settings, including hospitals, clinics, and home health agencies
  • Develop and implement individualized care plans that address the unique needs and goals of each patient
  • Collaborate with healthcare providers to ensure timely and effective care coordination
  • Provide education and support to patients and families to promote self-management and adherence to treatment plans
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
  • Communicate regularly with patients, families, and healthcare providers to ensure that care needs are met and expectations are managed
  • Supervise and manage care teams, ensuring that tasks are delegated effectively and efficiently

Requirements:

  • RN licensure in North Carolina
  • 2+ years of experience in nursing or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Proficiency in electronic medical records systems
