About the Job

We are seeking a skilled and compassionate Healthcare Coordinator to join our team at North Carolina Community Health Center Association.

Job Summary

The Healthcare Coordinator will play a crucial role in ensuring the coordination and continuity of healthcare services for patients. This individual will work closely with patients, caregivers, family members, primary care providers, specialists, and other healthcare professionals to provide comprehensive care management.

Responsibilities
  • Assess patient needs and develop personalized care plans to address medical, biopsychosocial, behavioral, spiritual, and cultural needs.
  • Collaborate with multidisciplinary teams to facilitate effective communication and ensure seamless care transitions.
  • Implement and monitor patient-centered interventions, set goals, and develop plans of care based on transitional care discharge plans/instructions.
  • Provide education to patients/families about clinical diagnosis, medications, available resources, prevention, and risk factors to achieve optimal self-management.
  • Monitor quality and effectiveness of interventions by setting SMART goals and collaborating with patients and families to identify goals.
  • Serve as an advocate and liaison among patients/families, community services, primary providers, specialists, and other care team members to coordinate services.
Requirements

To be successful in this role, the ideal candidate will possess:

  • An Associate degree or better from an accredited School of Nursing with an unrestricted license to practice nursing in NC.
  • A Certified Case Manager (CCM) certification is preferred.
  • Excellent written and oral communication skills.
  • Proficiency in Microsoft Office Outlook, Word, Excel, PowerPoint use, and e-mail communication.
  • Demonstrated ability to communicate clearly and succinctly.
  • Dependable, manages time well; efficient and organized.
  • Ability to produce accurate work; perform multiple tasks in a proficient and timely manner.
  • Good supervisory and management skills.

Light to moderate lifting is required.

Ability to uphold the stress of traveling.

Regular, predictable attendance is required.

Estimated Salary

$65,000 - $85,000 per year.
