Integrated Health Services Coordinator

We are seeking a highly skilled Integrated Health Services Coordinator to join our team at SouthLight Healthcare.
This role is a fantastic opportunity for individuals who are passionate about delivering exceptional patient care and making a positive impact in their community.
About the RoleThe successful candidate will be responsible for coordinating care services, establishing relationships with primary care providers, and addressing social determinants of health barriers that may hinder access to care.
You will work closely with our multidisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and delivery.
Key Responsibilities:- Establish relationships with primary care homes and become an expert in entrance criteria requirements
- Schedule appointments with participants and facilitate communication between healthcare providers
- Address social determinants of health barriers and provide case management services as needed
- Lead care coordination efforts between SouthLight Integrated Care Team and partner primary care homes
- Track and monitor CCBHC and other KPIs established by the organization
To be considered for this role, you should possess:
- A strong understanding of the healthcare system and its complexities
- Excellent communication and interpersonal skills
- Ability to develop and maintain effective relationships with patients, families, and healthcare providers
- Proficiency in Microsoft Office Suite and Electronic Health Records
- A degree in a relevant field (e.g., nursing, social work, or healthcare administration) or equivalent experience
We offer a competitive salary, starting at $18.00+ per hour, based on experience.
