About the Role

The Patient Care Coordinator will be responsible for coordinating patient care services, working closely with healthcare providers, patients, and families to ensure seamless transitions and high-quality care. This role requires strong organizational and time management skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Coordinate patient care services, including referrals, appointments, and follow-up visits.
  • Communicate effectively with patients, families, and healthcare providers to ensure accurate information exchange.
  • Work collaboratively with interdisciplinary teams to develop and implement comprehensive care plans.
  • Manage electronic health records, ensuring accuracy and timeliness of updates.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
Requirements
  • RN licensure in North Carolina required.
  • Certified Case Manager (CCM) certification preferred.
  • Excellent communication, organizational, and interpersonal skills.
  • Able to work in a fast-paced environment with multiple priorities.
  • Proficient in Microsoft Office Suite and electronic health records.
