Cancer Patient Navigator Job Summary

We are seeking an experienced Cancer Patient Navigator to join our team at MedStar Health. The successful candidate will be responsible for guiding patients and families throughout their cancer care journey, advocating for their needs, and facilitating timely access to necessary medical services.

Primary Duties and Responsibilities:
  1. Performs intake, assessment, and coordination of patient care
  2. Serves as a liaison for referring physicians and healthcare teams
  3. Screens patients for appropriate information and manages medical record acquisition
  4. Plans and implements patient evaluation processes
  5. Facilitates patient evaluation by interfacing with referring physicians and healthcare teams
  6. Coordinates complex patient management across various stages of diagnosis, treatment, and follow-up
  7. Navigates patients through the continuum of care, providing education and resources

Requirements:
  • Bachelor's degree in Nursing required
  • Master's degree preferred
  • 3-4 years clinical nursing experience required

Estimated Salary: $114,151 per annum.
