Job Summary

We are seeking a skilled RN Case Manager II to join our team at Community Memorial Healthcare. As a key member of our healthcare team, you will work collaboratively with patients, families, and healthcare professionals to develop and implement effective discharge plans and arrange post-acute care services.

Responsibilities
  • Assess patient needs and develop individualized care plans to ensure optimal outcomes
  • Collaborate with healthcare teams to coordinate post-acute care services, including home health, rehabilitation, and skilled nursing facilities
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless transitions and high-quality care
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes
  • Participate in quality improvement initiatives to enhance patient care and satisfaction
Requirements
  • Graduate of an accredited school of registered nursing
  • Current CA Registered Nurse (RN) License
  • Six (6) months of recent full-time acute care nursing experience in an acute care hospital (RN CM I)
  • One (1) year of case management experience (RN CM II)
Preferred Qualifications
  • Bachelors of Science in Nursing (BSN)
  • Bilingual in Spanish
  • One (1) year of experience in Case Management and strong discharge planning skills
  • One (1) year of experience with InterQual
  • One (1) year of experience with Allscripts and Meditech
