Job Summary

Responsibilities
  • Conduct thorough assessments of patients' needs and develop individualized care plans
  • Collaborate with interdisciplinary teams to ensure continuity of care and effective discharge planning
  • Coordinate with community resources and post-acute care providers to ensure timely and appropriate transitions
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements
  • Graduate of an accredited school of registered nursing
  • Current CA Registered Nurse (RN) License
  • At least 6 months of recent full-time acute care nursing experience in an acute care hospital
  • One year of case management experience preferred
