Sound Community Services, Inc. is seeking a dedicated Case Manager to join our Behavioral Health Home (BHH) team. As a Case Manager, you will play a vital role in supporting individuals with persistent mental illness and substance use disorders in achieving their recovery goals.

Key Responsibilities:

  • Assist individuals in accessing improved care coordination, linkage, referral, family support, and health and wellness promotion.
  • Follow up on care and ensure recommendations/referrals are implemented.
  • Assist with appointment scheduling and transportation to services.
  • Identify and address barriers to treatment participation.
  • Teach skills to help individuals maximize independence in the community.
  • Monitor participation in and response to treatment.
  • Regularly check in with individuals to assess their well-being.

Requirements:

  • Associate degree or 60 credits in a related field, or certification in a medical or human services field.
  • Minimum of two years of behavioral health or human services experience.
  • Valid Motor Vehicle Driver's license and vehicle available for use during the workday.
  • Competent user of technology, including email, agency intranet, and electronic client record.
  • Demonstrated ability to work with clients in the field teaching skills necessary for community living and supporting recovery.
  • Excellent written and oral communication skills.
  • CPR/First Aid Certified or eligible.
