Sound Community Services, Inc. is seeking a dedicated Case Manager to join our Behavioral Health Home (BHH) team. As a Case Manager, you will play a vital role in supporting individuals with persistent mental illness and substance use disorders in achieving their recovery goals.
Key Responsibilities:
- Assist individuals in accessing improved care coordination, linkage, referral, family support, and health and wellness promotion.
- Follow up on care and ensure recommendations/referrals are implemented.
- Assist with appointment scheduling and transportation to services.
- Identify and address barriers to treatment participation.
- Teach skills to help individuals maximize independence in the community.
- Monitor participation in and response to treatment.
- Regularly check in with individuals to assess their well-being.
Requirements:
- Associate degree or 60 credits in a related field, or certification in a medical or human services field.
- Minimum of two years of behavioral health or human services experience.
- Valid Motor Vehicle Driver's license and vehicle available for use during the workday.
- Competent user of technology, including email, agency intranet, and electronic client record.
- Demonstrated ability to work with clients in the field teaching skills necessary for community living and supporting recovery.
- Excellent written and oral communication skills.
- CPR/First Aid Certified or eligible.
