Overview

Hospice RN Case Manager

Job Summary

We are seeking a skilled RN Hospice Case Manager to join our team at Amedisys, a leading provider of home health and hospice care. As a RN Hospice Case Manager, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities

- Perform patient assessments and develop care plans in collaboration with the care team
- Make referrals to other hospice care disciplines as needed or document rationale for not doing so
- Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness
- Teach patients, families, and caregivers appropriate patient care techniques
- Regularly communicate with clinical manager and care team
- Perform on-call responsibilities and on-call services to patients/families as assigned

Qualifications

- One year of nursing experience
- Current RN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver's license, reliable transportation, and liability insurance
