Job Summary

Program Overview

The Population Health Program Manager is responsible for overseeing the development and implementation of the care management program for the University of Maryland Medical System's Population Health Services Organization. They design workflows to meet program goals and metrics, maintain systems for compliance with accrediting bodies, and establish relationships with various healthcare entities.

  • In collaboration with organizational leadership, the Manager develops and manages care management activities to improve population health outcomes and Value-Based Care initiatives.
  • Serves as a lead for Care Management in NCQA activities and collaborates with other departments to identify data needs for care management.
  • Manages the performance, consistency, and integration of care coordination services and supports an interdisciplinary practice environment.
  • Creates effective performance management systems for staff and contributes to performance reviews and documentation.

Key Responsibilities

Leadership and Collaboration

The successful candidate will demonstrate strong leadership and collaboration skills, with the ability to work effectively with cross-functional teams to achieve program goals.

  • Develops and implements care management strategies to improve population health outcomes.
  • Collaborates with healthcare providers, payers, and other stakeholders to ensure seamless care coordination.
  • Provides guidance and support to care management staff to ensure consistent delivery of high-quality care.

Operational Management

The Population Health Program Manager will be responsible for the day-to-day operations of the care management program, including:

  • Developing and managing budgets and resource allocation.
  • Ensuring compliance with regulatory requirements and accreditation standards.
  • Managing and analyzing data to inform program decisions and improve outcomes.

Qualifications

The ideal candidate will possess a strong background in healthcare operations, with experience in care management and population health. They will also have excellent leadership and collaboration skills, with the ability to work effectively in a fast-paced environment.

  • Master's degree in Healthcare Administration, Public Health, or related field.
  • Minimum 5 years of experience in care management or population health.
  • Proven track record of leadership and collaboration in a healthcare setting.
