We are seeking an experienced Nurse Practitioner to join our clinical team in Glendale, CA. This individual will provide in-home health assessment and patient care in the Glendale, CA area.
Key Responsibilities:- Visit members with complex care needs at home (6-8 members per day)
- Perform comprehensive health assessment, including obtaining history and performing physical examination, reviewing medications and related instructions, and reviewing home safety and psychosocial needs
- Fulfill documentation needs for STAR and HCC measures, obtain and document all pertinent information in patient's charts and case management notes on the same day as patient is seen
- Instruct all patients, caregivers, and family to call prior to accessing medical care, subject to life-threatening emergencies
- Maintain patient safety and confidentiality, and utilize universal precautions in all patient care
- Prescribe/order procedures, treatments, and medications within scope of practice, and rotate with the inpatient hospitalist's team for an assigned frequency and duration to be determined by the Campus Medical Director
- Evaluate patients at the Post Discharge Clinic with the attending Physician, and develop a high level of trust and intimacy with patients and families
