Job Title: Nurse Practitioner In Home Health

We are seeking an experienced Nurse Practitioner to join our clinical team in Glendale, CA. This individual will provide in-home health assessment and patient care in the Glendale, CA area.

Key Responsibilities:
  • Visit members with complex care needs at home (6-8 members per day)
  • Perform comprehensive health assessment, including obtaining history and performing physical examination, reviewing medications and related instructions, and reviewing home safety and psychosocial needs
  • Fulfill documentation needs for STAR and HCC measures, obtain and document all pertinent information in patient's charts and case management notes on the same day as patient is seen
  • Instruct all patients, caregivers, and family to call prior to accessing medical care, subject to life-threatening emergencies
  • Maintain patient safety and confidentiality, and utilize universal precautions in all patient care
  • Prescribe/order procedures, treatments, and medications within scope of practice, and rotate with the inpatient hospitalist's team for an assigned frequency and duration to be determined by the Campus Medical Director
  • Evaluate patients at the Post Discharge Clinic with the attending Physician, and develop a high level of trust and intimacy with patients and families

Requirements:
  • At least 1 year of prior experience
  • Graduation from an accredited Nurse Practitioner program
  • Board Certification (NP)
