We are seeking an experienced Nurse Practitioner to join our clinical team at Regal Medical Group. This individual will provide in-home health care, assessments, and patient care in assigned territories.
Key Responsibilities:- Visit members with complex care needs at home, conducting comprehensive health assessments and providing patient care.
- Perform physical examinations, review medications, and document patient information in charts and case management notes.
- Rotate with the inpatient hospitalist's team to maintain excellent clinical skills and evaluate patients at the Post Discharge Clinic.
- Develop a high level of trust and intimacy with patients and families, providing personalized care and support.
- At least 1 year of prior experience in home health care.
- Graduation from an accredited Nurse Practitioner program and Board Certification (NP).
- Competitive pay range of $130,000 - $140,000 annually.
