Job Summary

Regal Medical Group is seeking a highly skilled Nurse Practitioner to join our team as a Senior Wellness Specialist. In this role, you will be responsible for providing primary care to senior patients in a variety of settings, including our Senior Wellness Clinic, provider offices, and patient homes.

Key Responsibilities
  • Conduct annual wellness assessments for senior patients to identify health risks and develop personalized care plans.
  • Provide anticipatory guidance, screening, and health education to patients to promote healthy behaviors and prevent disease.
  • Assess senior patients for preventative interventions, such as safety, fall risk, depression screen, and urinary concerns.
  • Evaluate chronic conditions and develop plans to manage and improve patient outcomes.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Stay up-to-date with the latest clinical guidelines and best practices in gerontology.
Requirements
  • Active California Nurse Practitioner License
  • Active California RN License
  • Current BLS certification
  • Valid California Driver's License and proof of insurance
  • HCC experience
  • Demonstrated proficiency in use of Microsoft Office, Word, Excel, and other applications as required
  • Excellent observational, verbal, and written communication skills
  • Interpersonal skills necessary to work productively with patient, families, nursing staff, and other departments
  • Spanish speaking helpful
  • Three to Five years experience as a Nurse Practitioner - Preferred
