Job Summary:

The Hospice Nurse Navigator will serve as a liaison between case management, care providers, patients, and families, ensuring smooth and timely admission of eligible patients into hospice care at BWH. You will provide skilled assessment of patients to determine hospice eligibility, complete patient visits, and educate patients and families about hospice care.

Responsibilities:
  • Represent HopeHealth at BWH, serving as a hospice liaison between staff, patients, and families, and providing expert guidance on hospice care.
  • Conduct initial assessments of patients to determine eligibility for hospice care, considering their physical, functional, psychosocial, and environmental needs.
  • Present hospice care options to patients, families, and BWH staff, highlighting the benefits of hospice care in caring for individuals with a terminal diagnosis.
  • Complete hospice admissions and initiate care in compliance with Medicare and state regulations.
  • Document patient care and interactions with BWH staff in both HopeHealth and BWH electronic medical records.
  • Develop and implement individualized plans of care, incorporating interdisciplinary approaches and referrals to other disciplines as needed.
  • Ensure patient information regarding payer source is current and correct, working with BWH to obtain necessary insurance information and coverage.
  • Participate in weekly interdisciplinary team meetings to review patients receiving hospice inpatient care for 6 days or longer.
Requirements:
  • Strong desire to provide patient-centered care and a passion for end-of-life care.
  • Demonstrated commitment to quality end-of-life care and a strong clinical assessment and planning skills.
  • Bachelor of Science Degree in Nursing highly preferred.
  • Two or more years' experience in hospice, palliative care, and/or acute care.
  • Nursing license in Massachusetts and Rhode Island required; Massachusetts license required upon hire and evidence of Rhode Island license application required prior to start.
  • Individuals in this position will be required to achieve and maintain Hospice and Palliative Care certification through the Hospice and Palliative Nurses Association within three years of hire.
  • Excellent communication skills.
