Pharmaceutical Care Specialist

Position Summary
The Clinical Pharmacist at ATC White Plains will operate under the guidance of the Director of Pharmacy Services, taking charge of the oversight and administration of effective antibiotic treatment protocols within the healthcare facility. This role involves managing critical care and high-risk patient populations as designated by the specific area of responsibility. The Clinical Pharmacist will collaborate and consult with other healthcare professionals regarding the therapeutic application of antibiotics and other medications for patients within their purview. Additionally, this position entails providing educational support and training for healthcare staff engaged in this clinical setting.
Key Responsibilities Include:
1. Adhering to the established Performance Standards, Policies, and Behavioral Expectations of the organization.
2. Enhancing the monitoring and management of the clinical pharmacy antibiotic service or critical care/high-risk groups, implementing interventions aimed at improving the quality and cost-effectiveness of patient care.
3. Fostering the professional development of pharmacy staff by offering clinical leadership and educational opportunities as necessary.
4. Collaborating effectively with colleagues to achieve organizational goals, promoting positive interpersonal and team dynamics.
5. Generating innovative strategies and solutions to enhance performance, service quality, and overall patient care. This includes evaluating options and interventions, involving relevant stakeholders in decision-making, and making informed choices based on comprehensive information.
6. Taking personal accountability for all work-related activities and decisions, including following through on commitments and acknowledging errors without attributing blame to others.
7. Demonstrating respect and understanding towards colleagues by treating them courteously and maintaining effective working relationships.

14. Performing additional related duties as assigned.
Qualifications:
Required:
- Graduation from an accredited School of Pharmacy with a PharmD degree.
- Current pharmacist registration in New York State.
Preferred:
- Completion of a PGY1 residency with board certification; PGY2 residency is highly desirable.
Clinical Experience:
- Experience in managing patients in CCU/ICU settings.
- Expertise in counseling patients with CHF/COPD.
- Experience in investigating and reporting medication errors.
Core Competencies:
- Ability to interpret and communicate effectively in both written and verbal English.
- Professional demeanor and ability to serve as a role model.
- Ability to maintain composure under pressure.
Physical and Mental Requirements:
The role requires the ability to remain stationary or ambulate for extended periods, perform intricate manual tasks, and meet all regulatory requirements. The individual must focus on detailed tasks for extended durations and comprehend complex concepts while managing multiple responsibilities throughout the day.
Primary Population Served:
This position may involve working with diverse patient populations, including neonates, infants, children, adolescents, adults, and geriatric patients, as well as those with specific cultural and communication needs.
