Nurse Care Specialist Job Overview:

We are seeking an experienced Registered Nurse to join our Hospice team in San Antonio, TX. As a Hospice Nurse Care Specialist, you will provide high-quality patient care and coordinate the delivery of hospice services. Responsibilities include:
* Conducting comprehensive assessments of patients' physical, psychosocial, and environmental needs.
* Developing and implementing individualized care plans in collaboration with patients, families, and healthcare teams.
* Coordinating hospice services, including medical, emotional, and spiritual support.
* Maintaining accurate and up-to-date patient records and communicating effectively with patients, families, and healthcare teams.

The ideal candidate will have at least one year of clinical experience, current RN licensure in the state of practice, and current CPR Certification. A valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are also required. The salary for this position is estimated to be around $90,000 - $120,000 per year, depending on experience. LHC Group offers competitive compensation and benefits packages to its employees.
